Welcome to the Delta Dental Forever Green Kids Club, the Official kids supporters club of Seattle Sounders FC. Members will receive a Membership Card, discounted ticket offers, access to quarterly e-newsletters, and invitations to exclusive Member only events. By submitting this form you accept, on behalf of your child, Membership in the Delta Dental Forever Green Kids Club. All camp registrations will automatically be registered as members of the Forever Green Kids Club.

The primary, and in some cases exclusive method of communication for the Delta Dental Forever Green Kids Club is electronic mail. By accepting Membership on behalf of your child, you consent to receive all Delta Dental Forever Green Kids Club emails with exclusive Kids Club offers, announcements, and other Kids Club specific information, which may be sent from time to time to the email address you provided on this registration form. You acknowledge and agree that the Sounders may collect additional information from you or your child through his/her/their participation in the Kids Club. All information collected shall be subject to the MLS Privacy and Data policies, specifically as pertaining to minors and children as found here. We will always send you an email which will include that link and advising you about the nature and scope of the Kids Club which you have consented to receive. Once registered with Kids Club, we may send emails requesting general, non-personal information that requires each member to rate an experience, report their favorite player, favorite movie, or the like. You will always have the right to view your child's answers or request that any information related to your child's account is deleted or removed from our databases. Sometimes, we may request additional information in order for your child to participate in a specific activity or experience associated with a kid's targeted program or experience. We will only request information which is reasonably necessary to participate in the activity. From time to time, we contract with third party Service Providers to assist with the fulfillment of Kids Club programs and services. We may share personal information, including but not limited to the email address and name of the child associated with the account in order for these Service Provider to send content, messages or offers on our behalf associated with the Kids Club program or service. We will only share that information which is reasonably necessary to fulfill the program or service component. To the extent required by or permitted by law, we may also collect, use and disclose personal information in connection with security related law enforcement or to comply with legal requirements. Other than the Service Providers and law enforcement, we will not share your child's personal information with any third parties.

By accepting registration in the Kids Club, and subsequently participating at a Kids Club event or activity, you, on behalf of your child, forever release and grant to Seattle Soccer, LLC, the right in perpetuity to use any photographed or videotaped images and/or audio recording, personality and/or likeness, and any derivatives thereof of your child captured in connection with those Kids Club events or activities, in any medium, now or in the future, unless objected to or revoked by you in writing sent to Seattle Sounders FC, Attn: Kids Club Rights Release, 159 South Jackson Street, Suite 200, Seattle, Washington 98104. You have the right to review and update any email address that has been collected by the Delta Dental Forever Green Kids Club from or on behalf of your child and may elect to have such information deleted at any time. You may also unsubscribe to any future emails from Delta Dental Forever Green Kids Club by following the procedures through the "unsubscribe" hyperlink at the bottom of this email, OR in writing to Seattle Sounders FC, Attn: Kids Club Unsubscribe, 159 South Jackson Street, Suite 200, Seattle, Washington 98104. Note that if you choose to unsubscribe, you may not receive certain information about Kids Club program, events or activities and Sounders will not be required to provide an alternate method of communication for your child(ren) member(s). If you subsequently provide an additional or substitute email address for purposes of receiving Delta Dental Forever Green Kids Club email communications, including one belonging to your minor child, you additionally consent to receive all email communications at those addresses. All email addresses associated with your child's Delta Dental Forever Green Kids Club account shall be used solely for communications about the Kids Club and shall not be shared with any third parties.
